Chronic Disease Management

If you have a chronic disease you may be eligible for a  GP Management Plan. This will provide you with better health care.

If you have a medical condition that requires treatment from two or more health care providers, we can organise a Team Care Arrangement which entitles you to 5 Medicare sessions to allied health services (e.g. podiatry, dietician, exercise physiologist, physiotherapy), some of which could be at no out of pocket cost.

To receive a GP Management Plan and a Team Care Arrangement you will need to have a general assessment with the Doctor and Nurse who will develop an individualised plan suited to your personal health needs.

  1. It allows us allocated time to address your chronic health issues properly (rather than doing a catch up review during consults for another acute condition) – with more time we can deliver a better outcome for your health.
  2. Helps us organise tests and treatment in a timely fashion according to medical guidelines.
  3. Gives us allocated time to give you feedback

What is a chronic medical condition?

If your GP recognises that you have had a medical condition which has lasted more than six months then you could be diagnosed with having a chronic medical condition.

    • Diabetes
    • Asthma
    • Cancer
    • Heart Disease
    • Arthritis
    • Stroke
    • Other conditions could apply, speak to your doctor or nurse
